So around 3 months ago I came out as bisexual to my English teacher. She was the first person that I ever came out to and has been really supportive and comforting since the day that I met her. However at the end of the school year she is moving to another school and I'll never be able to see her again. I don't want her to leave without me being able to tell her what she means to me. I'd love to do something to show her how grateful I am. Any suggestions?

It sounds like this teacher is one of those people who has made an impact on your life that you will remember forever. I would say, don't over think it. Just think of some simple gesture that you could do to show her your appreciation: a letter, some cookies, a poem. I'm sure whatever you end up doing, she will understand how much she meant to you.
