Others can chime in but I don't think it's just anal sex that causes the orgasm, although for some it could be. I think for those that do it's more from the top hitting the bottoms prostrate or rubbing against it either during sex, fingering, or using a dildo.

It can and does. The perineum's rich in nerve bundles and also has to the prostate located a few inches above it. So some of the vibrations and pressure to the perineum also get transmitted to the prostate. It's actually a bit difficult to achieve as compared to the prostate orgasm via penetration because it's got thick skin over it. But once located and stimulated, the perineum's sweet spot increases in sensitivity with practice. I've even heard that it can even bring about the elusive multiple orgasm in guys. But no, it doesn't count as hands-free
I hate to nitpick, but I think by hands free, it means you are not actually touching your penis... I have achieved it once, and it was quite the experience.
It's extremely rare for most people - but yea it's from the prostate being stimulated not the actual act itself. It feels way better than a normal orgasm, and it's pretty unexpected. You can't really tell it's going to happen until it does so it catches you off guard with a nice surprise.
